Medical alert cards are a very useful tool to prevent breakdowns in communication during the post-accident chaos that occurs after an accident. This is the critical timeframe when first responders need to know the key aspects of your existing medical conditions and history that might be relevant to treatment options. Medical alert cards can provide this critical information.
As a public service, I have created a Medical Alert Card template and information form for you to use. It is easy to create the card and only takes a few minutes to complete. The template is a downloaded pdf fillable form which you can save to your computer. You can maintain and save a unique form for each family member. Once you have saved the form to your computer, you simply double click on the form icon and the template will automatically open using Adobe’s Acrobat Reader. Because the pdf template is a fillable form, you must have the free software from Adobe (if you do not have Acrobat Reader you can download this software free here). Follow the simple instructions to add the most important medical alert information, including a picture of the applicable family member. Print as many copies of the cards as you need — one for your wallet or purse, one in each car, or in the kid’s back-packs.
